Forms can be completed on your computer if you have the software and can use it, or printed and completed by hand. Please write legibly.

All forms and documents can be returned in these ways:

PLEASE KEEP A PHOTOCOPY OF ANYTHING YOU BRING OR SEND TO THE TRIBUNAL! MAIL AND DOCUMENTS CAN BE LOST!

Please note that the Basic Data Form must be completed in its entirety for it to be processed. If we receive an incomplete Basic Data Form, it will be returned to you, unprocessed. All of the blanks must be filled in with real information. Please do not put “do not know” or your form will be returned to you.
